RESUMO

Bronchiectasis is a clinical-radiological condition composed of irreversible bronchial dilation due to inflammation and infection of the airways, which causes respiratory symptoms, usually productive cough and infectious exacerbations. Bronchiectasis can have multiple causes, both pulmonary and extrapulmonary, and its clinical presentation is very heterogenous. Its prevalence is unknown, although up to 35-50% of severe COPD and 25% of severe asthma present them, so their underdiagnosis is evident. Chronic bacterial bronchial infection is common, and Pseudomonas aeruginosa is the pathogen that has been found to imply a worse prognosis. Treatment of bronchiectasis has three fundamental characteristics: it must be multidisciplinary (involvement of several specialties), pyramidal (from primary care to the most specialized units) and multidimensional (management of all aspects that make up the disease).

Introducción. Mycoplasma genitalium (MG) es un reconocido patógeno de transmisión sexual. El aumento de las resistencias asociadas a las principales líneas de tratamiento (macrólidos y quinolonas) justifican un estudio genético de mutaciones para mejorar las tasas de curación. Material y métodos. Un total de 8.508 muestras fueron analizadas entre abril de 2018 y julio de 2022 para la detección de MG mediante la técnica de PCR multiplex AllplexTM STI Essential Assay. En los casos positivos para MG se estudió el dominio V del gen 23S rRNA y los genes gyrA y parC. Se evaluó la importancia clínica de las mutaciones y se revisaron las historias clínicas para obtener información demográfica y de tratamiento. Resultados. Se realizó estudio de resistencias a 92 muestras (65 hombres y 27 mujeres). En lo relativo al estudio genotípico, 28 pacientes presentaban mutaciones a macrólidos (30,43%). La más habitual fue A2059G (18.48%). Para las quinolonas, 5 pacientes (5,43%) presentaron mutaciones clínicamente relevantes en el gen parC. Destaca un paciente con la mutación G295 en gyrA asociada a G248T en parC. Treinta individuos se sometieron al test de cura (TOC). La azitromicina fue el régimen empírico más común y el moxifloxacino la principal alternativa. Conclusiones. La elevada tasa de resistencias en nuestro entorno evidencia la necesidad de realizar una terapia dirigida por el estudio genotípico de resistencias a macrólidos, apoyándonos en la detección de mutaciones en parC y gyrA para predecir la susceptibilidad a quinolonas y en el uso del TOC para evaluar la respuesta al tratamiento (AU)


Introduction. Mycoplasma genitalium (MG) is a recognized sexually transmitted pathogen. Increasing resistance to main lines of treatment (macrolides and quinolones) justifies a genetic study of mutations to improve cure rates. Material and methods. A total of 8,508 samples from April 2018 to July 2022 were processed using AllplexTM STI Essential Assay. In MG positive cases 23S rRNA V domain, gyrA and parC genes were studied. Mutations detected were checked to assess their clinical significance and medical records were reviewed to obtain demographic and treatment information. Results. Resistance study was performed on 92 samples (65 men and 27 women). In relation to the genotypic study, 28 patients presented mutations to macrolides (30.43%). Most common was A2059G (18.48%). For quinolones, 5 patients (5.43%) had clinically relevant mutations in parC gene. Of note was a patient with G295 mutation in gyrA associated with G248T in parC. Thirty subjects underwent a test of cure (TOC). Azithromycin was the most common empirical regimen and moxifloxacin the main alternative. Conclusions. High rate of resistance in our environment evidences the need for targeted therapy by genotypic study of macrolide resistance, supported by the detection of mutations in parC and gyrA to predict quinolones susceptibility and the use of TOC to evaluate treatment response (AU)

La faringoamigdalitis es uno de los motivos más frecuentes de consulta en pediatría. Aproximadamente un 70-80% de las faringoamigdalitis son de etiología viral. El 20-30% restante son de origen bacteriano. El agente causal más frecuente es Streptococcus pyogenes (estreptococo ß-hemolítico del grupo A). El rol de Streptococcus dysgalactiae subsp. equisimilis, (estreptococos ß-hemolíticos grupos C y G) fue claramente establecido como agente etiológico en la faringitis bacteriana, tanto en niños como en adultos. Se realizó un análisis descriptivo y retrospectivo entre enero 2018 y diciembre de 2021. Se evaluó la prevalencia de faringitis estreptocócica, la edad, el período estacional, los agentes etiológicos y la resistencia a macrólidos durante los períodos pre-COVID-19 (2018-2019) y COVID-19 (2020-2021). Se analizaron 11 396 muestras de exudados de fauces de pacientes con sospecha de faringitis bacteriana; las mismas se procesaron mediante el uso de técnicas microbiológicas convencionales. En el período estudiado el porcentaje de positividad de los cultivos de exudados de fauces se mantuvo constante. Al comparar los períodos pre-COVID-19 (2018-2019) y COVID-19 (2020-2021) se observó una disminución en el número de aislados de S. pyogenes con un aumento de S. dysgalactiae subsp. equisimilis, mientras que la resistencia a macrólidos encontrada fue superior en S. pyogenes y para S. dysgalactiae subsp. equisimilis se mantuvo constante. Es importante realizar el cultivo para la identificación del agente etiológico y determinar la sensibilidad antibióticapara continuar con la vigilancia epidemiológica de la resistencia a los macrólidos, porque representan una opción en pacientes alérgicos a ß-lactámicos (AU)


Pharyngotonsillitis is one of the most frequent reasons for consultation in children. Approximately 70-80% of pharyngotonsillitis are of viral etiology. The remaining 20-30% are bacterial in origin. The most frequent causative agent is Streptococcus pyogenes (group A ß-hemolytic streptococcus). Streptococcus dysgalactiae subsp. equisimilis (ß-hemolytic streptococcus groups C and G) was clearly established as an etiologic agent in bacterial pharyngitis in both children and adults. A descriptive and retrospective analysis was conducted between January 2018 and December 2021. The prevalence of streptococcal pharyngitis, age, seasonal period, etiologic agents, and macrolide resistance during the pre-COVID-19 (2018-2019) and COVID-19 (2020-2021) periods were evaluated. We analyzed 11 396 specimens of swabs from patients with suspected bacterial pharyngitis. Conventional microbiological techniques were used. In the study period, the percentage of positivity of swab cultures remained constant. When comparing the preCOVID-19 (2018-2019) and COVID-19 (2020-2021) periods, a decrease in the number of S. pyogenes isolates was observed with an increase in S. dysgalactiae subsp. equisimilis, while the resistance to macrolides found was higher for S. pyogenes and remained constant for S. dysgalactiae subsp. equisimilis. The identification of the etiologic agent and determination of antibiotic sensitivity are important for epidemiological surveillance of macrolide resistance, as they are a treatment option in patients who are allergic to ß-lactams (AU)

Mycoplasma pneumoniae es una bacteria que carece de pared celular. Produce infecciones en todo el mundo, en brotes epidémicos cada 4-7 años, o en forma endémica. Sus manifestaciones clínicas se producen mayoritariamente en el tracto respiratorio y es una causa común de neumonía atípica. El tratamiento se realiza con macrólidos, tetraciclinas o fluoroquinolonas. Desde el año 2000 se ha detectado un aumento de resistencias a macrólidos a nivel mundial, siendo más frecuentes en Asia. En Europa la frecuencia de resistencias oscila entre el 1% y 25% en diferentes países. La combinación de técnicas moleculares y serológicas aporta una alta sensibilidad en la confirmación diagnóstica, siendo de gran utilidad para la detección y control de brotes de M. pneumoniae. La detección de resistencia a macrólidos requiere una técnica de secuenciación. (AU)


Mycoplasma pneumoniae is a bacterium that lacks a cell wall. It produces infections all It produces infections world-wide, in epidemic outbreaks every 4-7 years, or endemically. Its clinical manifestations occur mostly in the respiratory tract and it is a common cause of atypical pneumonia. The treatment is with macrolides, tetracyclines or fluoroquinolones. Since 2000, an increase in resistance to macrolides has been detected worldwide, being more frequent in Asia. In Europe the frequency of resistance ranges between 1% and 25%, depending on the country. Molecular techniques and serology techniques provides very high sensitivity in diagnostic confirmation, being very useful for detecting and controlling M. pneumoniae outbreaks. The detection of resistance to macrolides requires a sequencing technique.

Objectives: Mycoplasma genitalium causes persistent sexually transmitted infections. The aims of this study were to estimate the prevalence of resistances to macrolides and fluoroquinolones in M. genitalium and the sexually transmitted coinfections in patients at Hospital Universitario La Paz (Madrid, Spain). Material and methods: Patients attended between January and October 2021 were studied. Screening for sexually transmitted pathogens and detection of 23S rRNA and parC genes mutations were performed by real-time PCR (Allplex,SeegeneTM). Results: A total of 1,518 females and 1,136 males were studied. The prevalence of M. genitalium was 2.1%. The macrolides resistance rate was 51.8%. The mutations found were A2059G, A2058T and A2058G. The rate of resistance to fluoroquinolones was 17.8% being the G248T mutation (S83I) the most frequent. Seven males had some sexual transmitted coinfection. Conclusions: Although the percentage of M. genitalium infections is low, the high rate of resistance to macrolides makes it necessary to revise the protocols for diagnosis and empirical treatment of sexually transmitted infections. The use of fluoroquinolones is appropriate after screening of macrolide resistance profile. (AU)


Objetivos: Mycoplasma genitalium causa infecciones de transmisión sexual persistentes. Los objetivos de este trabajo fueron estimar la prevalencia de resistencias a macrólidos y fluoroquinolonas en M. genitalium así como las coinfecciones de transmisión sexual en pacientes del Hospital Universitario La Paz (Madrid, España). Material y métodos: Se estudiaron pacientes atendidos entre enero y octubre de 2021. El cribado de patógenos de transmisión sexual y la detección de mutaciones de los genes ARNr 23S y parC se realizaron por PCR en tiempo real (Allplex, SeegeneTM). Resultados: Se estudiaron 1.518 mujeres y 1.136 hombres. La prevalencia de M. genitalium fue del 2,1%. La tasa de resistencia a macrólidos fue del 51.8%. Las mutaciones encontradas fueron A2059G, A2058T y A2058G. La tasa de resistencias a fluoroquinolonas fue del 17.8% siendo la mutación G248T (S83I) la más frecuente. Siete hombres presentaron alguna coinfección de transmisión sexual. Conclusiones: Aunque el porcentaje de infecciones por M. genitalium es bajo, la elevada tasa de resistencias frente a macrólidos hace necesario modificar los protocolos de diagnóstico y tratamiento empírico de las infecciones de transmisión sexual. El uso de fluoroquinolonas es adecuado tras testar previamente el perfil de resistencia a macrólidos. (AU)

Introducción: Mycoplasma pneumoniae es causa frecuente de infecciones respiratorias en niños y jóvenes. Los macrólidos son la primera línea de tratamiento. La rápida emergencia de resistencia a estos antimicrobianos ha motivado el desarrollo de métodos moleculares para su detección en muestras clínicas positivas a este patógeno. Objetivo: Implementar un método de reacción en cadena de la polimerasa en tiempo real (RT-PCR) para la detección de resistencia a macrólidos a partir de muestras clínicas positivas a M. pneumoniae. Métodos: Se implementó una RT-PCR para la detección de las mutaciones A2058G y A2059G en el ARNr 23S de M. pneumoniae. Se analizaron 24 muestras clínicas positivas a M. pneumoniae, que provenían de pacientes con síntomas respiratorios. Se evaluó la sensibilidad, especificidad, repetibilidad y reproducibilidad de la RT-PCR. Resultados: La RT-PCR mostró un 100 % de especificidad para M. pneumoniae y un 92 % de sensibilidad, con un límite de detección de 2 copias/µL, que equivale a 10 copias/reacción. Además, se demostró la reproducibilidad y repetibilidad de estos resultados. Se obtuvo una correcta identificación de los genotipos salvaje y mutante, correspondientes a cada control. De las muestras clínicas positivas a M. pneumoniae, el 77,3 % (17/22) se identificó como sensible a macrólidos y el 22,7 % (5/22) como resistente. Conclusiones: La alta sensibilidad y especificidad del método de RT-PCR implementado permite que el Laboratorio Nacional de Referencia de Micoplasmas del Instituto de Medicina Tropical Pedro Kourí cuente con un método eficaz para el diagnóstico de M. pneumoniae resistente a macrólidos.


Introduction: Mycoplasma pneumoniae is a common cause of respiratory track infections in children and young adults. Macrolides are the first-line treatment. The rapid emergence of resistance to these antimicrobials has motivated the development of molecular methods for their detection in clinical samples positive for this pathogen. Objective: To implement a real-time polymerase chain reaction (RT-PCR) method for the detection of macrolide resistance in M. pneumoniae positive clinical samples. Methods: An RT-PCR was implemented to detect mutations A2058G and A2059G in 23S rRNA of M. pneumoniae. M. pneumoniae positive clinical samples from 24 patients with respiratory symptoms were analyzed. Sensitivity, specificity, repeatability and reproducibility of the RT-PCR assays were evaluated. Results: The RT-PCR assays showed 100% specificity to M. pneumoniae, and 92% sensitivity with a detection limit of 2 copies/µL, equivalent to 10 copies/reaction. Moreover, the repeatability and reproducibility of these results were demonstrated. Wild and mutant genotypes associated to each control were properly identified. Of the clinical samples positive for M. pneumoniae, 77.3% (17/22) were macrolide-sensitive and 22.7% (5/22) were macrolide-resistant. Conclusions: The high sensitivity and specificity of the RT-PCR method implemented provides the National Reference Laboratory of Mycoplasmas of the Institute of Tropical Medicine Pedro Kourí with an effective method for the diagnosis of macrolide-resistant M. pneumoniae.

Introducción. El uso inadecuado de antibióticos se asocia con aumento de la resistencia antimicrobiana, mayores costos de atención médica, más efectos adversos y peores resultados clínicos. Objetivo. Determinar los patrones de prescripción y las indicaciones aprobadas y no aprobadas para macrólidos en un grupo de pacientes en Colombia. Materiales y métodos. Se hizo un estudio de corte transversal sobre las indicaciones de uso de macrólidos en pacientes ambulatorios a partir de una base de datos de dispensación de medicamentos de 8,5 millones, aproximadamente, de personas afiliadas al sistema de salud de Colombia. Se consideraron variables sociodemográficas, farmacológicas y clínicas. Resultados. Se encontraron 9.344 pacientes que habían recibido prescripción de macrólidos; su mediana de edad era de 40,1 años (rango intercuartílico: 27,1-53,3 años) y el 58,3 % correspondía a mujeres. El macrólido más prescrito fue la azitromicina (38,8 %) y los usos más frecuentes fueron el tratamiento de la infección por Helicobacter pylori (15,9 %) y la neumonía (15,8 %). El 31,3 % de las prescripciones correspondía a indicaciones no aprobadas, destacándose el resfriado común (7,8 %), la Covid-19 (4,0 %) y la bronquitis aguda (3,5 %). La residencia en la región Caribe (OR=1,17; IC95% 1,05-1,31), las prescripciones odontológicas (OR=2,75; IC95% 1,91-3,96), las comorbilidades respiratorias crónicas (OR=1,30; IC95% 1,08-1,56), y la prescripción de eritromicina (OR=3,66; IC95% 3,24-4,14) o azitromicina (OR=2,15; IC95% 1,92 2,41), se asociaron con una mayor probabilidad de recibir macrólidos para indicaciones no aprobadas, en tanto que tener entre 18 y 64 años (OR=0,81; IC95% 0,71-0,93), 65 o más años (OR=0,77; IC95% 0,63-0,94) y residir en Bogotá-Cundinamarca (OR=0,74; IC95% 0,65 0,85) reducían dicho riesgo. Conclusiones. La mayoría de los pacientes recibieron macrólidos para infecciones del sistema respiratorio; la eritromicina y la azitromicina se prescribieron en indicaciones no aprobadas en menores de 18 años y en quienes presentaban enfermedades respiratorias crónicas.


Introduction: The inappropriate use of antibiotics is associated with a greater risk for antimicrobial resistance, high health care costs, adverse events, and worse clinical outcomes. Objective: To determine the prescription patterns and approved and nonapproved indications for macrolides in a group of patients from Colombia. Materials and methods: This was a cross-sectional study on the indications for the use of macrolides in outpatients registered in a drug-dispensing database of approximately 8.5 million people affiliated with the Colombian health system. Sociodemographic, pharmacological, and clinical variables were considered. Results: A total of 9.344 patients had received a macrolide prescription; their median age was 40.1 years (interquartile range: 27.1-53.3 years) and 58.3% were women. The most commonly prescribed macrolide was azithromycin (38.8%) most frequently for Helicobacter pylori infection (15.9%) and pneumonia treatment (15.8%). A total of 31.3% of the prescriptions were used for unapproved indications: common cold (7.8%), COVID-19 (4.0%), and acute bronchitis (3.5%). Residence in the Caribbean region (OR=1.17; 95%CI 1.05-1.31), dental prescriptions (OR=2.75; 95%CI 1.91-3.96), presence of chronic respiratory comorbidities (OR=1.30; 95%CI 1.08-1.56), and prescription of erythromycin (OR=3.66; 95%CI 3.24-4.14) or azithromycin (OR=2.15; 95%CI 1,92-2.41) were associated with a higher probability of macrolide use for unapproved indications while being 18-64 years old (OR=0.81; 95%CI 0.71-0.93) or 65 years or older (OR=0.77; 95%CI 0.63-0.94) and residing in Bogotá-Cundinamarca (OR=0.74; 95%CI 0.65-0.85) were associated with reduced risk. Conclusions: Most patients received macrolides for respiratory tract infections; erythromycin and azithromycin were used for unapproved indications in people under 18 years of age and those with chronic respiratory diseases.

Introducción: se describe a nivel mundial un aumento en la prescripción de macrólidos en niños y adolescentes, generando el riesgo de emergencia de cepas resistentes. Objetivo: caracterizar el uso de macrólidos en niños de 1 mes a 14 años hospitalizados en cuidados moderados e intensivos del Hospital Pediátrico del Centro Hospitalario Pereira Rossell (HP-CHPR). Metodología: estudio descriptivo transversal de niños hospitalizados tratados con macrólidos en el HP-CHPR en 2018. Variables: tipo de macrólido, duración del tratamiento, estudios y hallazgos microbiológicos y diagnóstico al egreso. Resultados: recibieron macrólidos 334 niños, mediana de edad 13 meses, 58,4% varones. 71,0% en Unidad de Terapia Intensiva (UTI). Predominó la prescripción de claritromicina (72,8%), durante los dos últimos cuatrimestres del año (77,5%) y por patología respiratoria (94%): bronquiolitis (23,3%), infección aguda no especificada de las vías respiratorias inferiores (21,9%) y crisis asmática (19,1%). Mediana de tratamiento con azitromicina y claritromicina 5 y 8 días respectivamente. Se realizaron estudios microbiológicos en 96,1% sin determinarse microorganismo en 58,3%. Conclusiones: se destaca el uso de macrólidos principalmente en la UTI y por patología respiratoria. La prescripción por fuera de las recomendaciones nacionales vigentes y la baja confirmación microbiológica que apoye el uso fueron los mayores problemas detectados, por lo que parece fundamental establecer estrategias tendientes a promover un uso racional de estos antibióticos.


Introduction: literature has described a global increase in the prescription of macrolides to children and adolescents , which has increased the risk of emergence of resistant strains. Objective: to characterize the use of macrolides in children from 1 month to 14 years of age hospitalized at the moderate and intensive care units of the Pereira Rossell Pediatric Hospital Center (HP-CHPR). Methodology: descriptive cross-sectional study of hospitalized children treated with macrolides at the HP-CHPR in 2018. Variables: macrolide type, treatment duration, microbiological studies and findings, and diagnosis at discharge. Results: 334 children received macrolides, median age 13 months, 58.4% males. 71.0% hospitalized atnan Intensive Care Unit (ICU). Clarithromycin was mainly prescribed in 72.8% of the cases, during the last two quarters of the year (77.5%), due to respiratory disease (94%): bronchiolitis (23.3%), lower respiratory tract unspecified acute infection (21.9%) and asthma crisis (19.1%). Median treatment included Azithromycin and Clarithromycin for 5 and 8 days respectively. Microbiological studies were carried out in 96.1% of the cases and 58.3% did not show the presence of microorganisms. Conclusions: the use of macrolides stands out, mainly at ICUs and due to respiratory pathologies. The main problems identified were prescriptions made outside the framework of the present national recommendations and the low microbiological confirmation for their use, which suggests it is essential to set strategies to promote a more rational use of these antibiotics.


Introdução: a literatura descreve um aumento a nível global na prescrição de macrolídeos para crianças e adolescentes, o que tem aumentado o risco de surgimento de cepas resistentes. Objetivo: caracterizar o uso de macrolídeos em crianças de 1 mês a 14 anos de idade internadas nas unidades de terapia moderada e intensiva do Centro Hospitalar Pediátrico Pereira Rossell (HP-CHPR). Metodologia: estudo transversal descritivo de crianças hospitalizadas tratadas com macrolídeos no HP-CHPR em 2018. Variáveis: tipo de macrolídeo, duração do tratamento, estudos e achados microbiológicos e diagnóstico no momento da alta. Resultados: 334 crianças receberam macrolídeos, idade mediana de 13 meses, 58,4% do sexo masculino. 71,0% internados em Unidade de Terapia Intensiva (UTI). A Claritromicina foi prescrita principalmente em 72,8% dos casos, nos últimos dois trimestres do ano (77,5%), devido a doença respiratória (94%): bronquiolite (23,3%), infecção aguda não especificada do trato respiratório inferior (21,9%) e crise de asma (19,1%). O tratamento médio incluiu Azitromicina e Claritromicina por 5 e 8 dias, respectivamente. Estudos microbiológicos foram realizados em 96,1% dos casos e 58,3% não evidenciaram a presença de microrganismos. Conclusões: destaca-se o uso de macrolídeos, principalmente em UTIs, e devido a patologias respiratórias. Os principais problemas identificados foram as prescrições feitas fora das atuais recomendações nacionais e a baixa confirmação microbiológica para sua utilização, o que sugere que é essencial definir estratégias para promover uma utilização mais racional destes antibióticos.

RESUMEN El objetivo del estudio fue identificar molecularmente los genes de virulencia y resistencia a macrólidos en aislamientos clínicos de Streptococcus agalactiae (EGB), recuperados en 2019 a partir de secreción vaginal (n=9) y orina (n=22), en dos establecimientos de salud de Lima. La identificación y susceptibilidad antimicrobiana se determinaron por el sistema automatizado Vitek® 2, se confirmó la identificación fenotípicamente; la resistencia a macrólidos por el método D-test; la identificación de genes de virulencia (lmb, bca y rib) y de resistencia a macrólidos (ermB, ermTR y mefA) por reacción en cadena de la polimerasa (PCR). El fenotipo y genotipo de resistencia a macrólidos predominante fue cMLSb (12/31) y ermB (11/31), y el gen de virulencia más frecuente fue lmb (23/31). Todos fueron sensibles a penicilina, ampicilina y vancomicina. Estos hallazgos muestran la necesidad de implementar estudios de epidemiología molecular que permitan un adecuado conocimiento y seguimiento de EGB en el Perú.


ABSTRACT The aim of the study was to molecularly identify virulence and macrolide resistance genes in clinical isolates of Streptococcus agalactiae (GBS), recovered in 2019 from vaginal discharge (n=9) and urine (n=22), from two health facilities in Lima. Identification and antimicrobial susceptibility were determined by the Vitek® 2 automated system, identification was confirmed phenotypically; macrolide resistance was determined by the D-test method. Identification of virulence genes (lmb, bca and rib) and macrolide resistance genes (ermB, ermTR and mefA) was carried out by polymerase chain reaction (PCR). The predominant macrolide resistance phenotype and genotype were cMLSb (12/31) and ermB (11/31); the most frequent virulence gene was lmb (23/31). All were sensitive to penicillin, ampicillin and vancomycin. These findings show the need to implement molecular epidemiology studies that allow adequate knowledge and follow-up of GBS in Peru.

Although the chronic presence of microorganisms in the airways of patients with stable chronic obstructive pulmonary disease (COPD) confers a poor outcome, no recommendations have been established in disease management guidelines on how to diagnose and treat these cases. In order to guide professionals, the Spanish Society of Pulmonology and Thoracic Surgery (SEPAR) has prepared a document which aims to answer questions on the clinical management of COPD patients in whom microorganisms are occasionally or habitually isolated. Since the available scientific evidence is too heterogeneous to use in the creation of a clinical practice guideline, we have drawn up a document based on existing scientific literature and clinical experience, addressing the definition of different clinical situations and their diagnosis and management. The text was drawn up by consensus and approved by a large group of respiratory medicine experts with extensive clinical and scientific experience in the field, and has been endorsed by the SEPAR Scientific Committee.

INTRODUCTION: Mycoplasma genitalium is a major cause of urethritis and other genital syndromes. Antibiotic resistance, especially to macrolides, is increasing at an alarming rate worldwide. The aim of this study was to estimate the rate of macrolide resistance in M. genitalium among a 2016-2017 cohort of patients in Barcelona, Spain; and to compare this estimate with previous data from 2013 to 2014 in this region. METHODS: The study was conducted retrospectively with M. genitalium-positive samples collected between December 2016 and February 2017 at the Hospital Vall d'Hebron Microbiology Department. Genotypic markers of macrolide resistance were primarily detected using the ResistancePlus® MG molecular assay (SpeeDx). Mutations were then confirmed by sequencing. RESULTS: Macrolide resistance-mediating mutations were detected in 30/83 infections (36.1% [95% CI, 25.9%-47.4%]). This resistance was more frequent among men who have sex with men (55.0% [95% CI, 38.5%-70.7%]) compared to heterosexual men (27.3% [95% CI, 10.7%-50.2%]) and women (9.5% [95% CI, 1.3%-30.4%]), p<0.001. Additionally, macrolide resistance did not significantly increase in this cohort when compared with previous investigations. CONCLUSION: Despite the current notable rate of macrolide resistance in M. genitalium, resistance did not significantly increase between 2013-2014 and 2016-2017 in our region. Nevertheless, strict local surveillance and the implementation of rapid diagnostic tests that combine the detection of the bacterium and resistance-mediating mutations may facilitate the optimization of antibiotic administration and reduce the transmission of resistance in M. genitalium.

RESUMEN OBJETIVO: Ureaplasma urealyticum es el agente más frecuentemente aislado en infección intraamniótica. Los macrólidos son los antimicrobianos de primera elección en embarazadas. Se ha descrito el aumento de resistencia, pudiendo limitar las opciones terapéuticas durante la gestación. El propósito del estudio es evaluar susceptibilidad antimicrobiana de Ureaplasma urealyticum aislado en mujeres en edad fértil, que se atienden en Clínica Alemana Temuco, Araucanía, Chile. METODO: Se estudian todas las muestras de orina y flujo vaginal para cultivo de U. urealyticum, de pacientes entre 18 y 40 años, recibidas en el Laboratorio de Microbiología Clínica Alemana Temuco, en período Abril 2013 a Enero 2015. Se procesan las muestras con kit Mycoplasma IST 2 de Biomerieux. En las que resultan positivas, se estudia susceptibilidad a macrólidos, tetraciclinas y quinolonas. RESULTADOS: 426 muestras de orina y flujo vaginal (390 pacientes). 197 pacientes resultaron positivas para U. urealyticum. (50,5%). La susceptibilidad fue 88,4% (174 pctes) a Eritromicina, 87,9% (173 pctes) a Claritromicina y 91,9% (181 pctes) a Azitromicina (NS). 15 de 197 pacientes (7,6%) fueron resistentes a los 3 macrólidos. La susceptibilidad a Quinolonas fue 55,3% a Ciprofloxacino, y 94% a Ofloxacino. El 100% resultó susceptible a Tetraciclinas. CONCLUSIONES: Cerca del 10% de U. urealyticum aislados en nuestra serie son resistentes a macrólidos, contribuyendo a la no erradicación de la infección en tratamientos empíricos. Dentro de ellos, azitromicina aparece con la mayor efectividad. El aumento de resistencia limitará opciones terapéuticas, con gran impacto perinatal en futuro. La vigilancia de susceptibilidad en cada hospital es fundamental para elección terapéutica.


ABSTRACT INTRODUCTION: Ureaplasma urealyticum is the most frequently isolated microorganism in intra-amniotic infection. The macrolides are the first choice antimicrobials for treat this infection in pregnancy. The increasing resistance has been described worldwide, seriously limiting therapeutic options in pregnancy. The aim of the study is to evaluate antimicrobial susceptibility of U. urealyticum aislated in fertile-age women in Clínica Alemana Temuco, Araucania region, Chile. METHOD: Urine and vaginal samples were analyzed for U. urealyticum, from every 18 to 40 years old patients, received at Microbiology Laboratory of Clínica Alemana Temuco, between April 2013 to January 2015. The samples are processed with Mycoplasma IST 2 kit of Biomerieux. If they became positives, susceptibility to macrolides, tetracyclines and quinolones was studied. RESULTS: 426 urine and vaginal samples were collected (390 patients). 197 patients were positive for U. urealyticum (50.5%). The susceptibility was 88.4% (174 pts) to Erythromicyn, 87.9% (173 pts) to Clarithromycin and 91.9% (181 pts) to Azithromycin (NS). Resistance to all macrolides was observed in 15 out of 197 patients (7.6%). The susceptibility to Quinolones was 55.3% to Ciprofloxacin, and 94% to Ofloxacin. The 100% was susceptible to Tetracyclines. DISCUSSION: Near to 10% of isolated Ureaplasma spp in our serie were resistant to some macrolide, being a factor for failing to eradicate the infection in empirical treatment. Azithromycin was the most effective. The increasing resistance will limit therapeutic options, with great perinatal impact in the future. Susceptibility surveillance in each hospital is very important for therapeutic options.

INTRODUCTION AND OBJECTIVES: Mycoplasma pneumoniae (MP) is one of the most common etiological agents of community-acquired pneumonia (CAP) in children. We aimed to describe the clinical and epidemiological characteristics, treatment and outcome of children diagnosed with community-acquired MP pneumonia (CAMP) in a tertiary hospital in Valencia, Spain. MATERIAL AND METHODS: Medical records of children <14 years with CAMP were retrospectively reviewed from January 2010 to December 2015. Patients with radiological evidence of pneumonia and microbiological confirmation of MP (PCR from nasopharyngeal swab and/or serum specific IgM) were considered CAMP. RESULTS: One hundred and sixty two children were diagnosed with CAMP; median age 6 years (IQR: 4-9). The positive MP test rate among children with CAP progressively increased with age as did the empirical use of macrolides. There were two peaks of cases in 2011 and in 2015, being July, August, November and December the seasons with the higher number of cases. The most frequent radiological pattern was segmental infiltrate (62.3%) and 22 (13.6%) children had pleural effusion. It was noteworthy the mild symptomatology and low levels of inflammatory parameters that children with CAMP had. A macrolide was empirically initiated in 68.5% of cases. Hospital admission rate was inversely proportional to patient's age. CONCLUSIONS: According to this study, older, less symptomatic patients and with lower inflammatory parameters had the greatest rate of MP infection among children with CAP and thus they could benefit of empiric macrolide therapy. Therefore, knowing the epidemiology of a geographical area may be important for the management of CAP in children.

In 2008, the Spanish Society of Pulmonology (SEPAR) published the first guidelines in the world on the diagnosis and treatment of bronchiectasis. Almost 10 years later, considerable scientific advances have been made in both the treatment and the evaluation and diagnosis of this disease, and the original guidelines have been updated to include the latest therapies available for bronchiectasis. These new recommendations have been drafted following a strict methodological process designed to ensure quality of content, and are linked to a large amount of online information that includes a wealth of references. The guidelines are focused on the treatment of bronchiectasis from both a multidisciplinary perspective, including specialty areas and the different healthcare levels involved, and a multidimensional perspective, including a comprehensive overview of the specific aspects of the disease. A series of recommendations have been drawn up, based on an in-depth review of the evidence for treatment of the underlying etiology, the bronchial infection in its different forms of presentation using existing therapies, bronchial inflammation, and airflow obstruction. Nutritional aspects, management of secretions, muscle training, management of complications and comorbidities, infection prophylaxis, patient education, home care, surgery, exacerbations, and patient follow-up are addressed.

La linfadenitis es la manifestación clínica más frecuente de la infección por micobacterias no tuberculosas en niños inmunocompetentes. Se presentan dos casos de linfadenitis por M. lentiflavum diagnosticados en un hospital de tercer nivel en los últimos 10 años. Se realizaron pruebas complementarias de rutina ante adenopatía persistente y se obtuvo una muestra mediante drenaje para el cultivo, que resultó positivo para este germen. Ambos pacientes recibieron tratamiento antibiótico oral durante varias semanas. El caso 1 precisó exéresis completa al quinto mes de evolución, mientras que el caso 2 presentó resolución completa de la lesión a los 4 meses. M. lentiflavum es considerado, de entre las nuevas especies de micobacterias no tuberculosas recientemente descritas, un germen emergente en nuestro medio. Posee unas características microbiológicas y clínicas especiales, diferentes del resto de las micobacterias no tuberculosas. Son pocos los casos publicados hasta la fecha desde que se describió por primera vez la infección en 1997.


Lymphadenitis is the most common clinical feature in nontuberculous mycobacterium infection in immunocompetent children. We present two case reports of M. lentiflavum lymphadenitis diagnosed in a tertiary hospital in the last 10 years. Routine tests were performed after persistent adenopathy, and a sample for culture was obtained, being positive for this microorganism. Both patients received oral antibiotics during several weeks. Case 1 needed complete excision after five months of treatment, whilst Case 2 was cured by medical therapy. M. lentiflavum is considered, among the newly described nontuberculous mycobacterial species, an emergent pathogen in our environment. It has its own microbiological and clinical characteristics, different from the rest of nontuberculous mycobacteria. Case reports are limited in the literature since the infection was described for the first time in 1997.

La enfermedad pulmonar obstructiva crónica es un proceso morboso diseminado, con importantes implicaciones clínicas y económicas. Se realizó una revisión bibliográfica de los artículos científicos publicados en PubMed, Scielo y MEDLINE, entre los años 2010 al 2015, acerca del tratamiento inhalatorio de la enfermedad pulmonar obstructiva crónica, principales tratamientos, reacciones adversas y los logros de la terapéutica moderna. Recientes ensayos clínicos han llevado a la aprobación de nuevas terapias inhalantes para esta enfermedad. Muchos de estos medicamentos son combinaciones de beta agonistas de larga acción y anti colinérgicos de larga acción, para la fase tardía de la enfermedad pulmonar obstructiva crónica. Se reconoce la necesidad de más investigaciones que establezcan comparaciones entre las opciones terapéuticas disponibles y la relevancia clínica de las nuevas opciones, para que éstas se traduzcan en una mejoría del volumen de expiración forzado (AU)


Chronic obstructive pulmonary disease is a disseminated process with important clinical and economic implications. A bibliographic review of scientific articles published in PubMed, Scielo and MEDLINE between 2010 and 2015 was carried out concerning the inhalation treatment for the chronic obstructive pulmonary disease COPD), its main indications, adverse reactions and achievements of modern therapy. Recent clinical trials have led to the approval of new inhalation therapies for the disease. Many of these medicaments are combinations of long-acting beta antagonists and long-acting anticholinergics for the late stage of the COPD. The necessity of more investigations capable to establish comparisons between the available treatment options and the clinical relevance of new alternatives to improve the forced expired vital capacity is acknowledged (AU)

Introducción: las infecciones por S. aureus meticilino resistentes son un problema de salud pública por el perfil de multirresistencia que presenta este patógeno. Objetivo: determinar el fenotipo de resistencia a meticilina, macrólidos y lincosamidas en cepas de S. aureus. Materiales y métodos: se analizaron 50 cepas de S. aureus aisladas de muestras clínicas de pacientes del Hospital Rosario Pumarejo de López en la ciudad de Valledupar. Las pruebas de susceptibilidad a meticilina, eritromicina y clindamicina se realizaron por los métodos microdilución en caldo y difusión en agar. Se determinó la resistencia a meticilina mediante la técnica agar dilución y la resistencia inducible a clindamicina, con la prueba del D-Test. Resultados: la resistencia a meticilina fue del 50%, se evidenciaron cinco fenotipos en los macrólidos y lincosamidas analizados: el fenotipo con sensibilidad a eritromicina y clindamicina (78%), fenotipo con resistencia a eritromicina y clindamicina (16%), que presentan resistencia constitutiva para ambos antimicrobianos MLSBc, liderando los fenotipos de resistencia; el fenotipo con sensibilidad intermedia a ambos antimicrobianos (2%), el fenotipo con resultado intermedio para eritromicina y sensibilidad a clindamicina (2%) y el fenotipo con resistencia a eritromicina y sensibilidad a clindamicina (2%), que presentan resistencia inducible a clindamicina MLSBi, con prueba test D positiva. Conclusiones: la resistencia inducible para macrólidos, lincosamidas y streptograminas no se detecta usando los test de susceptibilidad antimicrobiana estándar. La no identificación de esta resistencia inducible puede conducir a falla del tratamiento con clindamicina.


Introduction: Infections with methicillin-resistant S. aureus are a public health problem due to the multi-resistance profile presented by this pathogen. Objective: To determine resistance phenotypes to methicillin, macrolides and lincosamides in S. aureus. Materials and methods: 50 S. aureus strains, isolated from patients of the Hospital Rosario Lopez Pumarejo in the city of Valledupar, were analyzed. Susceptibility tests to methicillin, erythromycin and clindamycin were performed using microdilution and agar diffusion methods. Methicillin resistance was determined through agar dilution technique and inducible clindamycin resistance D-Test. Results: Methicillin resistance reached 50%, five phenotypes were established in the analyzed macrolides and lincosamides: phenotype sensitive to erythromycin and clindamycin (78%); phenotype resistant to erythromycin and clindamycin (16%) with constitutive resistance for both cMLSB antimicrobials, which lead the resistance phenotypes; phenotype with intermediate resistance to both antimicrobials (2%); the intermediate result phenotype resistant to erythromycin and clindamycin (2%); and the RS phenotype resistant to erythromycin and sensitive to clindamycin (2%) that show inducible iMLSB clindamycin resistance with positive D test. Conclusions: The inducible resistance to macrolides, lincosamides and streptogramines is not established through the standard antimicrobial susceptibility test. Not identifying the inducible resistance can lead to clindamycin treatment failure.


Introdução: As infeções por S. aureus meticilino resistentes são um problema de saúde pública pelo perfil de multirresistência que apresenta este patógeno. Objetivo: Determinar o fenótipo de resistência à meticilina, macrolídeos e lincosamidad em cepas de S. aureus. Materiais e métodos: Analisaram-se 50 cepas de S. aureus isoladas de amostras clínicas de pacientes do Hospital Rosario Pumarejo de López na cidade de Valledupar, Colômbia. A susceptibilidade à meticilina, eritromicina e clindamicina se realizaram pelos métodos microdiluição em caldo e difusão em ágar. Determinou-se a resistência à meticilina mediante a técnica ágar diluição e a resistência induzível a clindamicina, com a prova D-Test. Resultados: A resistência a meticilina foi de 50%, se evidenciaram cinco fenótipos nos macrolídeos e lincosamidas analisados: o fenótipo com sensibilidade à eritromicina e clindamicina (78%), fenótipo com resistência à eritromicina e clindamicina (16%), que apresentam resistência constitutiva para ambos os antimicrobianos MLSBc, liderando os fenótipos de resistência; o fenótipo com sensibilidade intermedia a ambos os antimicrobianos (2%), o fenótipo com resultado intermédio para a eritromicina e sensibilidade à clindamicina (2%) e o fenótipo com resistência a eritromicina e sensibilidade à clindamicina (2%), que apresentam resistência induzível à clindamicina MLSBi, com prova test D positiva. Conclusões: A resistência induzível para macrolídeos, lincosamidas e streptograminas não se detecta usando os testes de susceptibilidade antimicrobiana standard. A não identificação desta resistência induzível pode conduzir à falha do tratamento com clindamicina.

En esta nueva edición de Ronda Clínica y Epidemiológica analizamos cinco estudios que consideramos importantes para la actualidad de la práctica clínica. El estudio RENALRIPC en el cual Zarbock y colaboradores sugieren que el acondicionamiento isquémico remoto previo a cirugía cardiovascular reduce el riesgo de presentar lesión renal aguda. El estudio CAP-START en el que Postma y colaboradores muestran que el uso de monoterapia con betalactámicos no es inferior a la combinación de estos con macrólidos o a las fluoroquinolonas para prevenir mortalidad a 90 días en pacientes con neumonía adquirida en la comunidad. El estudio STOPAH revela que ni la pentoxifilina ni la prednisolona parecen tener efecto significativo en la mortalidad a 28 días de los pacientes con hepatitis alcohólica grave. Andersson y colaboradores presentan un estudio en el cual se evidencia que postergar al menos 3 minutos el pinzamiento del cordón umbilical al nacimiento puede significar un mejor desarrollo cognitivo a los 4 años de edad, en comparación con el pinzamiento temprano. Finalizamos con una nota de prensa del Instituto de Alergias y Enfermedades Infecciosas de Estados Unidos sobre el estudio START, mostrando que el inicio de la terapia antirretroviral en pacientes con infección por VIH cuando el recuento de CD4+ es mayor de 500 células/μL reduce significativamente el riesgo de desenlaces adversos, comparado con aquellos en los que se inicia cuando el recuento de CD4+ es de 350 células/μL o menos.


In this new edition of Ronda Clínica y Epidemiológica, five studies that we consider important in today's clinical practice are analyzed. The RENALRIPC study, in which Zarbock et al., suggest that remote ischemic preconditioning previous to cardiovascular surgery reduces the risk of acute kidney injury. The CAP-START study, in which Postma et al., show that the use of monotherapy with beta-lactams was not inferior to their combination with macrolides or to the use of monotherapy with fluoroquinolones in reducing 90-days mortality in patients with community acquired pneumonia. The STOPAH study, in which neither pentoxifylline nor prednisolone demonstrated a significant effect in preventing 28-day mortality in patients with severe alcoholic hepatitis. Andersson et al., present a study which shows that delaying the clamping of the umbilical cord may lead to a superior cognitive development at the age of four, compared with those in which the clamping was done earlier. Finally, we include an extraordinary press report of the National Institute of Allergy and Infectious Diseases regarding the START study, showing that the start of antiretroviral treatment in patients infected with HIV when the CD4+ cell count is higher than 500/μL significantly reduces the risk of adverse outcomes, when compared with those in which therapy is started when CD4+ cell count declined to 350/μL or less.


Nesta nova edição de Ronda Clínica e Epidemiológica analisamos cinco estudos que consideramos importantes para a atualidade da prática clínica. O estudo RENALRIPC no qual Zarbock e colaboradores sugerem que o acondicionamento isquêmico remoto prévio a cirurgia cardiovascular reduz o risco de apresentar lesão renal aguda. O estudo CAP-START no que Postma e colaboradores mostram que o uso de monoterapia com beta-lactâmicos não é inferior à combinação destes com macrólidos ou às fluoroquinolones para prevenir mortalidade a 90 dias em pacientes com pneumonia adquirida na comunidade. O estudo STOPAH revela que nem a pentoxifilina nem a prednisolona parecem ter efeito significativo na mortalidade a 28 dias dos pacientes com hepatite alcoólica grave. Andersson e colaboradores apresentam um estudo no qual se evidência que postergar ao menos 3 minutos o estrangulamento do cordão umbilical ao nascimento pode significar um melhor desenvolvimento cognitivo aos 4 anos de idade, em comparação com o estrangulamento precoce. Finalizamos com uma nota de imprensa do Instituto de Alergias e Doenças Infecciosas de Estados Unidos sobre o estudo START, mostrando que o início da terapia antirretroviral em pacientes com infecção por HIV quando a recontagem de CD4+ é maior de 500 células/μL reduz significativamente o risco de desenlaces adversos, comparado com aqueles nos que se inicia quando a recontagem de CD4+ é de 350 células/μL ou menos.

Introduction: Streptococcus agalactiae (GBS) is the most common agent in early neonatal sepsis. Strategies incorporating universal screening for maternal colonization show the lowest rates of perinatal infection. A significant increase in resistance to erythromycin and clindamycin by GBS has been reported around the world. There are no published data regarding prevalence and antimicrobial resistance in southern regions of Chile. Surveillance of antimicrobial resistance is essential to define the drugs of choice and alternatives, in an institution that applies prevention protocols, as Clinica Alemana Temuco (CAT) does. Objectives: to determine the prevalence of carriage of GBS in vaginal-anal areas at end of pregnancy, in CAT, Araucanía Region, Chile. To determine the susceptibility to erythromycin and clindamycin of GBS strains isolated. Results: 1,181 pregnant women were included; 167 were positive for GBS (14.4% of colonization). Sixteen were resistant to erythromycin (9.5%); 15 of these strains were also clindamycin resistant. Twenty-three of 167 were resistant to clindamycin (13.7%). Conclusions: The prevalence rate of GBS colonization was lower than previously reported in other regions of Chile. Due to the high rates of resistance to clindamycin and erythromycin it is necessary to widen the study of susceptibility to other antimicrobials to have alternatives in allergy to penicillin (primarily cefazolin and vancomycin).


Introducción: Streptococcus agalactiae es el más frecuente causal de sepsis neonatal precoz. Las estrategias con pesquisa universal de colonización materna muestran las tasas más bajas de infección perinatal. Se ha reportado un significativo aumento de resistencia de S. agalactiae a eritromicina y clindamicina en el mundo. No existen datos publicados de prevalencia y susceptibilidad antimicrobiana en las regiones del sur de Chile. La vigilancia de resistencia es fundamental para definir los antimicrobianos de elección y alternativas para prevención del cuadro en instituciones que apliquen estrategias de prevención, como Clínica Alemana Temuco (CAT). Objetivos: Determinar la prevalencia de portación de S. agalactiae en la región vaginal-anal de mujeres embarazadas de tercer trimestre en CAT. Determinar la susceptibilidad a eritromicina y clindamicina de las cepas aisladas. Resultados: 1.181 embarazadas fueron incluidas. 167 resultaron S. agalactiae (+) (14,4% de colonización). Diez y seis eran resistentes a eritromicina (9,5%). Quince de ellas también lo eran a clindamicina. Veintitrés de 167, eran resistentes a clindamicina (13,7%). Conclusiones: La tasa de prevalencia de colonización (14%) fue más baja que lo reportado anteriormente en el centro del país. Debido a la alta tasa de resistencia a clindamicina y eritromicina se hace necesario aumentar el estudio de susceptibilidad a otros antimicrobianos alternativos en pacientes alérgicas a penicilina (principalmente cefazolina y vancomicina).

RESUMO

Torsade de pointes is a frequent cause of sudden death of cardiac origin, with uncertain mechanisms of actions and very diverse origins. A case is presented of a patient with a bronchial condition on pharmacological treatment with macrolides, and who, as well as having atrial fibrillation, suffered an episode of self-limiting torsade de pointes that abated spontaneously with no associated clinical complications.
